New applicants for operators permit will need your original birth certificate, social security card, and proof of address. If you are under the age of 18, you will also need your no pass/no drive form from your school or board of education along with a parent or legal guardian to sign. If you wish to take the CDL written test please choose a earlier test time to give yourself time to complete all test required & ensure you have all your documents on the CDL Portal submitted, at www.mycdl.ky.gov . This would include your application, self-certification and current medical certificate. This process can take up to 7-10 business days before it is complete. Hazmat applicants, in addition to the CDL Portal submissions, must also make sure the fingerprint background has been returned. CDL license holders will have to pay $5 for each endorsement test you take. If you want to take the written test to obtain your motorcycle permit, you must at least have your intermediate license. If you are under 18, a parent or legal guardian will have to be with you to sign. If you are getting a reinstatment because of a suspended license you will first need to pay your $40 reinstatement fee online at www.drive.ky.gov or by calling the DOT Regional Field office at 270-746-7493.
