PLEASE DO NOT SCHEDULE EXTRA/MULTIPLE APPOINTMENTS FOR THE SAME TEST. There still exists a backlog in some areas, and we will delete and reassign any duplicates found. If you are found to have multiple appointments, only the earliest appointment by date and time will be honored. You will be required to wear a mask before entering the building. New applicants will need to bring an original birth certificate, social security card, and proof of address. If under the age of 18, you will also need your no pass/no drive form from your school or board of education along with a parent or legal guardian to sign for you. When scheduling your appointment, please enter a complete first and last name. At the discretion of this office, we will delete and reassign any incomplete appointments. • CDL Written Test – Please ensure all required documents have been submitted on the CDL Portal. This includes your application, self-certification and current medical certificate. This process typically takes 7-10 business days to complete. Hazmat applicants must also make sure the fingerprint background has been returned. Call (888) 655-9655 to schedule your appointment to be fingerprinted. New CDL applicants will be required to pay a $24.00 application fee before testing. CDL license holders will have to pay $5 for each endorsement test you take. • Motorcycle Permit – An intermediate license, at least, is required. If under the age of 18, a parent or legal guardian will have to be with you to sign. • Reinstatement – Must first pay $40.00 reinstatement fee online at www.drive.ky.gov • Non-Permanent Resident – Start with Circuit Court Clerk or local Transportation Cabinet Office. Visit www.drive.ky.gov for additional info. • Verify required documents at www.drive.ky.gov. Failure to bring all required documents may result in cancelation of your appointment.
